Stop and Watch

San Jacinto Manor uses the INTERACT Program. Part of this program is “Stop and Watch”.

The “STOP and WATCH” Tool is an “early warning” tool to identifying any common, but nonspecific changes in condition a patient is having. This tool can be used by any staff member who gives direct care, non-direct care (eg, housekeeping, dietary, rehabilitation, office, etc.) and by any family members. This tool is an alert for nursing to determine if further evaluation is necessary. The form has two copies, one is given to the patient’s charge nurse and the other is given to the Director of Nurses. You can find the tool around the home on the lined carts, at the nurse’s station, in the front office and more.
